Job summary

Elara Caring is seeking a Clinical Team Manager to oversee patient care services, staff assignments, and care coordination across our Home Health teams in Indiana. You will ensure timely starts of care, regulatory compliance, and high-quality documentation.

The role requires RN licensure, 2 years in home health, and 1 year of supervisory experience, with proficiency in Homecare Homebase and Workday. Travel between patient homes is required.

Qualifications

• Associate degree in nursing; a bachelor’s degree is preferred.
• 2 years of home care experience in Home Health or Hospice.
• 1 year of supervisory experience.
• Current, unrestricted RN license valid for Indiana.
• Proficiency with Homecare Homebase, Workday, and Microsoft Office.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance, coaching, and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

What is Required?

• Associate degree in a Nursing related field, Bachelor preferred
• 2 years of home care experience in Home Health or Hospice environment
• 1 year of supervisory experience required
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
