Job summary

AccentCare Inc. is seeking a full-time Hospice Clinical Liaison - RN in the Atlanta, GA area (including Gwinnett County). The role covers clinical liaison duties, with a Monday–Friday schedule and limited on-call responsibilities.

As a clinical liaison, you will educate providers, coordinate with Admissions and Team Directors, and collaborate with Hospice Care Consultants on marketing plans. Qualified candidates will have RN experience, familiarity with Medicare and home health regulations, and

Qualifications

• One (1) year of RN experience in local market with knowledge of government home health regulations, Medicare requirements, and care plan development techniques.
• Previous medical sales experience preferred.
• Licensed RN currently licensed as a registered nurse in the state of operation.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Work with Hospice Care Consultants to develop and implement individualized marketing plan.
• Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
• Act as a liaison with Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families.
• Make inpatient visits at contracted facilities and communicate with Admissions Department, Team Directors and/or Primary Nurse.
• Provide consultation on hospice and palliative care.

Be the Best Clinical Liaison RN You Can Be

If you meet these qualifications, we would love to meet you:

• One (1) year of RN experience in local market with a strong knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques

• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order
