Hospice Clinical Liaison serves as the first introduction to hospice and palliative care for hospitalized patients and their families. The Hospice Clinical Liaison educates patients and families on hospice care and provides ongoing support. Working collaboratively with the patient, family, and physician, the Clinical Liaison helps develop a care plan aligned with their wishes. He/she conducts nursing assessments, makes judgments, and treatment decisions based on patient needs. The Clinical Liaison initiates assessments to ensure the care plan's appropriateness and adherence.
Schedule: Monday - Friday 8:30am - 5:00pm
Location: Falls Church, VA

In collaboration with the hospice and palliative care medical consultant, the Clinical Liaison initiates care planning and discharge planning, and communicates with the interdisciplinary team regarding patient goals and treatment plans. The Clinical Liaison promotes Capital Caring and Capital Palliative Care Consultants programs to acute care facilities through collaborative relationships, providing opportunities for professional development and community education. May be asked to rotate to other regions or departments.
Experience Requirements
Minimum of one year of experience in a comparable setting or unit. Hospice and/or palliative care experience is strongly preferred.
Education Requirements
Degree in Nursing from an accredited nursing program. A BSN degree is preferred.
Required Certificates and/or Licenses
Licensure from the governing body for practice in the District of Columbia, Maryland, and/or Virginia, depending on the state of practice. Licensure from a compact state is acceptable.
Driver's License
Valid driver's license in good standing and access to a vehicle during working hours.
