Overview
This position will cover Washington D.C.
We're looking for a Hospice Clinical Liaison LPN/LVN to join our team. You will report directly to the AVP of Business Development. You will be responsible for providing ongoing information on the Company's hospice services and referral coordination. Participating in patient and family informational meetings and upon receipt of a referral, meet with the patient and family to provide information regarding hospice services.
Provide education about hospice, clinical outcomes and hospice benefits to referral sources.
Use superior clinical, customer service, and sales skills and knowledge to attract and maintain business relationships in the health care industry.
Inform hospital personnel of case acceptance and document all demographic, clinical and payer information.
Maintain an in-depth knowledge of compliance with all local, state and federal laws related to home health regulations, Medicare requirements, and care plan development techniques.
About You
Graduate of an approved school of Practical/Vocational Nursing
Minimum two years LPN/LVN experience, in oncology, long-term care, hospice or home health
Three to five years' experience preferred
