Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Client Care Coordinator (CCC). You will support a full range of patient services to bring comfort and dignity to our clients, emphasizing timely enrollment and coordinated care.

The role requires strong organizational skills, communication with families and providers, and collaboration with RN and pharmacy teams. We offer competitive pay, benefits, Daily Pay, and a supportive, equal-opportunity work environment.

Qualifications

• High School graduate or equivalent.
• One year of related healthcare experience is required.
• Experience in working in the senior field or hospice is preferred.
• Must have a valid driver's license.

Responsibilities

• Facilitate new client enrollment during initial and follow-up screenings and maintain client health records.
• Identify resources to address client’s health, daily living, and safety needs.
• Schedule, log, and document all appointments and services.
• Communicate with clients by phone and computer.
• Coordinate RN and pharmacy medication set-ups and reminders.
• Serve as family liaison coordinating with families, designees, and providers.

As a Hospice Client Care Coordinator (CCC), you will:

• Facilitate new client enrollment within initial and subsequent screening assessments and further maintain client’s healthcare record

• Perform administrative tasks such as scheduling, logging, documenting all appointments and services
• Communicate to clients through telephone and computer
• Coordinate with RN and pharmacy medication set-ups and reminders
• Be a family liaison and coordinate with the family, designees, and providers
