Job summary

Anchor Health seeks a Certified Home Health Aide with Hospice experience to join our San Diego team. You will provide compassionate in-home care, including hygiene, mobility support, meal prep, and daily living activities.

The role requires thorough charting, timely documentation, and close coordination with the Case Manager. A California driver’s license and willingness to travel are required.

Qualifications

• CHHA certified required or eligible
• Hospice care experience preferred
• BLS certification required
• Ability to communicate effectively, both verbally and in writing
• Bi-lingual abilities considered an asset
• Current California driver’s license and auto insurance proof

Responsibilities

• Completes charting and timely documentation.
• Assists patients with bathing, mouth care, skin, hair and personal hygiene.
• Assists patients with transferring and ambulation as needed.
• Assists with prescribed exercises as taught by health professionals.
• Prepares meals and assists patients with eating.
• Assists patients with toileting including bedpans, urinals, and commodes.
• Prepares household services to support self-care at home and prevent institutionalization.
• Reports changes in patient condition to Case Manager or Team Manager.
