Job summary

Anchor Health is seeking a Certified Home Health Aide with Hospice experience to join our San Diego team, serving communities in the Livermore, Pleasanton, Dublin, San Ramon area. The role involves compassionate in-home care, charting duties, and patient support across basic daily activities.

Requirements include a CHHA program completion, 1–2 years of patient care, BLS certification, and willingness to travel. Bilingual is a plus; competitive pay and benefits are offered.

Qualifications

• High School diploma or equivalent and CHHA program completion.
• 1–2 years direct patient care in a clinical or home setting.
• BLS certification required; strong communication skills.
• Travel required; bilingual is a plus.

Responsibilities

• Complete charting and timely documentation.
• Assist with bath, hygiene, and personal care.
• Assist with transferring and ambulation as needed.
• Support prescribed exercises as directed by health professionals.
• Prepare meals and assist with eating.
• Assist with toileting including bedpans and commodes.
• Prepare household services to support self-care at home.
• Report changes to the Case Manager or Team Manager.
