Job summary

Astrana Health Management seeks a compassionate Certified Home Health Aide (CHHA) to join our hospice interdisciplinary team in Alhambra, CA. This part-time field‑based role provides ADL assistance, emotional support, and care coordination across the SGV region.

The CHHA will document care in HospiceMD and travel between patient locations. The ideal candidate holds a CA CHHA license, CPR, and English proficiency; Mandarin/Cantonese is strongly preferred.

Qualifications

• Certified Home Health Aide (CHHA) license required in CA.
• CPR certification required.
• Mandarin or Cantonese preferred to serve SGV patients, but English required.

Responsibilities

• Assist with ADLs: bathing, dressing, grooming, feeding, oral hygiene.
• Provide skin care, safe ambulation, transfers, and prescribed exercises.
• Monitor vital signs and report changes to Case Manager.
• Log visits and care plans in HospiceMD per policy.
• Travel between SGV patient locations and submit mileage via Concur.
• Collaborate with nurses, social workers, chaplains, and families to carry out the plan of care.
