Job summary

Swan Hospice is looking for a Certified Home Health Aid (CHHA) with hospice experience to join their dedicated team in Long Branch, NJ. The ideal candidate will assist in providing quality hospice care, ensuring respect and dignity for patients in various settings.

The role includes assisting with personal care, light housekeeping, and reporting vital signs to the nursing team. Required qualifications include an active HHA license in NJ and a minimum of one year's experience in hospice care.

Qualifications

• HHA License in the state of NJ.
• Minimum of one year experience working in hospice.
• Reliable and able to understand the hospice philosophy.

Responsibilities

• Assist with prescribed exercise and ROM activities.
• Provide assistance in toileting, positioning, and transfer activities.
• Assist with ADL activities such as personal hygiene and grooming.
• Help patients take medications as per regulations.
• Perform light housekeeping duties when needed.
• Assist with feeding and meals.
• Obtain vital signs as directed by the hospice nurse.
• Report changes in patients' conditions to the nurse.
