Job summary

Ascend Hospice & Palliative Care NJ is hiring a Certified Home Health Aide (CHHA) to provide compassionate in-home care. You’ll assist with ADLs, personal hygiene, grooming, and mobility, under supervision of a Hospice Nurse.

Candidates must have a State Certified CHHA license; prior hospice or home health experience preferred; willingness to travel; valid driver’s license and reliable transportation; HS diploma. Join a team dedicated to dignity and quality of life.

Qualifications

• Must have a State Certified CHHA license.
• Experience in hospice or home health preferred.
• No experience necessary but 1 year preferred.
• Must be willing to travel and have reliable transportation.
• High School Diploma or equivalent required.

Responsibilities

• Provide or assist with ADL activities and personal hygiene.
• Assist with toileting, mobility, transferring, and positioning.
• Support prescribed exercises and ROM under supervision.
• Light housekeeping of patient unit and laundry as needed.
• Assist with meals and basic nutrition support.
• Record vitals (temperature, pulse, respiration) as directed.
• Report changes to the Hospice Nurse and document appropriately.
• Assist to take medications in compliance with regulations.
