Job summary

Swan Hospice is seeking a Certified Home Health Aide (HHA) / CNA with hospice experience to join our compassionate team in New Jersey. Ideal candidates will hold a valid HHA license in NJ and demonstrate a strong understanding of hospice philosophy.

We require at least 1 year of hospice experience and dependable transportation to serve residents in facilities and private homes. This role focuses on dignity, comfort, and high-quality patient care across settings.

Qualifications

• HHA license in NJ is required.
• Hospice philosophy understood.
• Minimum 1 year hospice experience.
• Dependable transportation required.

Responsibilities

• Assist with prescribed exercises and ROM under Hospice Nurse supervision.
• Assist with toileting, positioning, transfers, and turning of bed-bound residents/patients.
• Assist with ADL: hygiene, grooming, mouth, skin care, nails.
• Assist or remind patient to take medications per regulatory and accrediting bodies.
• Light housekeeping of resident's unit/room; change bed linens; patient laundry.
• Feed and assist with meals.
• Take temperature, pulse and respiration as supervised by Hospice Nurse.
• Report changes in resident/patient condition to Hospice Nurse and document time/date.
