Job summary

Swan Hospice is seeking a CHHA with hospice experience to join our team. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities, and private homes.

Ideal candidates will have an NJ HHA license, understand the hospice philosophy, and at least one year of hospice experience, with dependable transportation to reach patients across our service area.

Qualifications

• HHA license in NJ is required.
• Understands hospice philosophy and care approach.
• Minimum of 1 year experience in hospice care.
• Dependable transportation to visit patients.

Responsibilities

• Assist with prescribed exercises and ROM as taught by Hospice Nurse.
• Provide or assist with toileting, transfers, and positioning for bed-bound residents.
• Assist with ADLs: hygiene, grooming, skin care, nails.
• Remind or assist with medication per regulations.
• Light housekeeping of the patient’s unit and laundry.
• Assist with meals and feeding as needed.
• Take basic vitals as supervised by the Hospice Nurse.
• Report changes to Hospice Nurse and document reports time/date and recipient.
