Job summary

Mission Healthcare in Ventura County, CA, seeks a full‑time Chaplain/Spiritual Care Provider to deliver compassionate spiritual support to hospice patients and families. You will conduct visits, offer prayer and counseling, and develop personalized care plans within an interdisciplinary team.

Qualifications

• Master of Divinity or equivalent theological degree from an accredited institution.
• Ordination or endorsement from a recognized religious body.
• Previous hospice, hospital, or pastoral care experience preferred.
• Strong interpersonal, communication, and counseling skills.
• Ability to work with diverse religious, cultural, and spiritual backgrounds.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide spiritual and emotional support to hospice patients and their families.
• Conduct regular visits with patients, family members, and caregivers to assess spiritual needs.
• Offer prayer, counseling, and support based on individual needs.
• Develop and implement individualized spiritual care plans.
• Support interdisciplinary team members with spiritual insights into care.
• Conduct memorial services, grief support groups, and related services as needed.
• Respect diverse cultural, spiritual, and religious practices and traditions.
• Collaborate with social workers, nurses, physicians, and other care team members.
• Provide bereavement support to families post-death, including follow-up calls and visits.
• Maintain accurate documentation of spiritual care interactions and outcomes.
