Lakes Region Visiting Nurse Association is seeking a Chaplain (Hospice)/Bereavement to provide spiritual care across the Lakes Region of central New Hampshire. This is a part-time, per diem role serving patients and families with sensitivity to diverse faiths.

The hospice chaplain will assess psycho-spiritual needs, collaborate with an interdisciplinary team, and support grief and end-of-life care. A Master of Divinity and ACPE‑accredited CPE are required, along with a valid driver's license.

Qualificações

• Master of Divinity (M.Div.) or equivalent from an accredited seminary/theological school.
• Minimum of two CPE units from an ACPE‑accredited program.
• Ordained, commissioned, or recognized to function in pastoral care.
• Minimum of one year of hospice or end-of-life care experience; two years preferred.
• Strong spiritual assessment skills for grief, loss, and trauma.
• Experience in multi-faith settings or related background is recommended.
• Proficiency in using EMR systems.
• Valid driver’s license with reliable vehicle insurance.

Responsabilidades

• Provide spiritual support and counseling to hospice patients and families.
• Conduct spiritual assessments to identify psycho-spiritual and emotional needs.
• Collaborate with the hospice team to create and implement spiritual care plans.
• Offer empathetic care to those dealing with life-limiting illnesses, loss, and grief.
• Facilitate spiritual rituals, prayers, or sacraments per patient/family requests.
• Document spiritual care activities in the medical record per policy.
• Participate in team meetings to discuss patient care.
• Provide crisis intervention during trauma, grief, and end-of-life transitions.
• Liaise with patient, family, and community spiritual resources.
• Maintain competency through continuing education and training.
