Job summary

Tech9 is seeking a Hospice Spiritual Care Counselor to visit patients in their homes and oversee spiritual care aligned with patients’ beliefs and care plans.

Responsibilities include conducting spiritual assessments, developing care plans with the IDG, collaborating with clergy, and maintaining clinical documentation. You will participate in IDG/QAPI activities and may conduct services or bereavement support as needed.

Qualifications

• Ordained, commissioned or credentialed according to religious group.
• Degree in theology or seminary required or preferred.
• Experience supporting spiritual needs in hospice or healthcare settings.
• Comfortable working in an interfaith environment.

Responsibilities

• Conduct spiritual assessments within 5 days of admission.
• Develop and implement the interdisciplinary patient care plan for spiritual care.
• Provide spiritual support aligning with patient/family beliefs and wishes.
• Collaborate with local clergy when desired by the patient.
• Maintain and submit clinical records including evaluation, care plan and daily notes.
• Document visits and interventions in the patient record; coordinate with Case Managers.
• Participate in IDG and QAPI meetings and activities.
• Conduct religious services for patients/families as appropriate.
• Provide bereavement support to staff and family; educate IDG on spiritual care.
• Offer funeral or memorial services for patients when requested.

HOSPICE SPIRITUAL CARE COUNSELOR that visits patients and their families in their homes.DUTIES OF POSITIONProvide counseling or arranges counseling to provide and/or oversee meet the spiritual care of the patients/ family members/caregivers in a manner consistent with their beliefs and desires and in accordance with the patient’s plan of care. Ensuring that patient’s needs are met and rights are preserved.POSITION RESPONSIBILITIESConduct a spiritual assessment as part of the comprehensive assessment within 5 days of admission to the hospice program.Assist in development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care.Provide spiritual support in accordance with on-going wishes and needs of the patient/family in accordance with their religious preferences or definition of spirituality.Work in close collaboration with local clergy when desired by the patient.Maintain and submit written clinical records as deemed by the Agency, including the initial evaluation, the care plan and daily notes.Documents visits, observations and interventions in the patient’s clinical record including any contact with spiritual caregivers outside the interdisciplinary team.Evaluate the patients and family's response to, and effectiveness of, the spiritual counseling.Confirm, on a weekly basis, the scheduling of visits with the patients’ Case Managers to facilitate coordination of other staff visits.Participate in IDG and QAPI meetings and activities.Conduct religious services of prayer, worship and rituals for patients/families, as appropriate.Working with other professionals in resolving spiritual/ethical issues.Provide spiritual and bereavement support to staff as needed.Provide consultation and education to the IDG on spiritual care.Provide funeral or memorial services for patient’s as requested.Assume responsibility for self-development by continually striving to improve his/her Spiritual practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.JOB CONDITIONSThe ability to access patients’ homes which may not be routinely wheelchair accessible is required.Physical activities will include, walking, sitting, stooping, and standing.EQUIPMENT OPERATIONUtilization of computer, multi-line telephone, copy machine and other office equipment.COMPANY INFORMATIONHas access to patient medical records.QUALIFICATIONSOrdained, commissioned or credentialed according to the practices of an organized religious group.ORHas a degree in theology or from a seminary.Demonstrated experience and training to support the spiritual needs of the patient and family.Comfortable in an interfaith setting.Hospice experience preferred.Able to accept different lifestyles, cultures, beliefs and values.Must be a licensed driver with an insured automobile in good working order.The ability to communicate well, both verbally and in writing.You'll NeedTB testPhysical Form Signed ( we provide the form)CPR CertificationImmunization records2 Forms of IDReliable TransportationAuto InsuranceE04JI800p5k8409b4wh
