Crossbridge Hospice, LLC is seeking a Chaplain to provide spiritual and bereavement support to patients and families. You will work with an interdisciplinary team to develop individualized care plans that honor each patient’s spiritual journey.

Ideal candidates hold a Bachelor’s degree in a related field, have at least one year of hospice experience, and possess strong communication skills. A Master’s degree is preferred; driving is required to serve families across the community.

Formación

• Bachelor’s degree in religious studies, social work, psychology, counseling, pastoral education, or a related field.
• Strong written and verbal communication skills.
• Ability to honor and affirm the religious and spiritual autonomy of all individuals without imposing personal beliefs.
• Availability during agency operating hours and as needs arise.
• Valid driver’s license and good driving record.

Responsabilidades

• Provide spiritual and bereavement counseling to hospice patients and families.
• Participate in interdisciplinary group (IDG) meetings and maintain open communication with team.
• Uphold patient confidentiality and ethical standards in all interactions.
• Facilitate connections to community spiritual resources when appropriate.
• Preside over funerals or memorial services at family requests.
• Support Age‑Friendly Care by integrating the 4Ms into spiritual care.
