Job summary

Bristol Hospice in Harrisburg, PA seeks an experienced Hospice Chaplain to join a growing organization. You will meet patients and families with spiritual assessments, develop an individualized plan of care, and collaborate with the Interdisciplinary Team.

Responsibilities include direct spiritual care, liaison with community chaplains, documentation, and participation in care conferences. Travel to hospice locations and flexible hours are required; hospice experience is preferred.

Qualifications

• Graduate theological degree (BD/M.Div. or equivalent).
• Experience with death and dying; death-care experience preferred.
• Hospice experience preferred.
• Willing to travel to hospice locations; flexible working hours.
• Proficient with Word, Excel, PowerPoint and EMR systems.

Responsibilities

• Assure spiritual assessment of patients and families; plan care accordingly.
• Develop individualized care plans based on spiritual needs.
• Provide direct spiritual care to patients and families.
• Liaise with community chaplains and spiritual counselors.
• Document services and communicate with the care team.
• Attend care conferences and interdisciplinary meetings.
• Recruit community chaplains and volunteers.
• Provide memorial services as requested.
