Job summary

Angels Care Hospice seeks a Hospice Chaplain to deliver spiritual care to patients, families, and caregivers, coordinating with the interdisciplinary team. The role emphasizes compassionate assessment, planning, and direct support to those facing end-of-life journeys.

You will collaborate with community chaplains, document services, participate in quality initiatives, and offer education to staff. Funeral and memorial services may be requested.

Qualifications

• Graduate of accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).

Responsibilities

• Assesses spiritual needs of patients and families/caregivers and ensures timely services.
• Participates in comprehensive assessments to meet spiritual needs and develops individualized care plans.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison and support to community chaplains and spiritual counselors.
• Maintains records of spiritual care services and related activities for quality assurance and review.
• Attends patient care conferences and interdisciplinary group meetings as a member of the team.
• Provides consultation, education, and support to the interdisciplinary group on spiritual care.
• Recruits community chaplains and volunteers to meet patient and family needs and educates congregations about hospice care.
• Provides funeral or memorial services as requested and plans periodic memorial services for staff and volunteers.
• Assists in supervision of spiritual care volunteers and participates in quality improvement activities.
