Job summary

Bristol Hospice - Madison Llc is seeking an experienced Hospice Chaplain to support patients and families through spiritual assessments and care planning as part of the Interdisciplinary Team. You will work closely with clinicians to ensure compassionate, dignified care.

You will provide direct spiritual care, document activities, and collaborate with community chaplains and volunteers. Travel to hospice locations is required, with a values-driven, patient-centered approach to end-of-life

Qualifications

• Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred.
• Must have experience working with death and dying, individuals/family/caregiver.
• Hospice experience preferred.
• Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.

Responsibilities

• Assure spiritual assessment of patients and families in the hospice program and ensure services are rendered in a timely manner.
• Facilitate development of the individualized plan of care by participating in comprehensive assessment to meet identified spiritual needs.
• Provide direct spiritual care to patients and families/caregivers.
• Serve as liaison and support to community chaplains and spiritual counselors.
• Maintain records of spiritual care services utilization and related activities for QA and program development.
• Attend patient care conferences and interdisciplinary group meetings as a member of the team.
• Recruit community chaplains, spiritual counselors and volunteers to meet patient needs; promote education in congregations.
• Provide for funeral or memorial services for patients as requested.
