Job summary

Bristol Hospice - Chicago Llc is seeking an experienced Hospice Chaplain to lead spiritual assessments, contribute to individualized plans of care, and provide direct spiritual support to patients and families in the hospice program.

You will collaborate with the interdisciplinary team, coordinate with community chaplains, and help ensure compassionate, dignified end-of-life care across assigned locations. Willingness to travel within the service area is expected.

Qualifications

  • Graduate of an accredited seminary or theology school.
  • Experience working with death and dying; hospice experience preferred.
  • Ability to provide direct spiritual care and participate in the care team.

Responsibilities

  • Conduct spiritual assessments for patients and families.
  • Develop individualized plans of care addressing spiritual needs.
  • Provide direct spiritual care to patients and families.
  • Coordinate with community chaplains and spiritual counselors.
  • Maintain records of spiritual care activities for QA and compliance.
  • Attend patient care conferences and interdisciplinary meetings.
