Job summary

Angels Care Hospice seeks a chaplain to deliver spiritual care to patients, families, and caregivers, coordinating with church and community chaplains. The role involves assessment, plan development, documentation, and collaboration with an interdisciplinary team.

The chaplain will recruit volunteers, provide education, and may conduct funeral or memorial services as requested, ensuring compassionate support across the hospice program.

Qualifications

• Graduate of accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).

Responsibilities

• Provide spiritual care to patients, families, and caregivers, directly or via coordination with chaplains.
• Develop individualized plans of care reflecting spiritual needs and participate in assessments.
• Document services and maintain records for quality assurance and program review.
• Attend care conferences and interdisciplinary meetings as a team member.
• Recruit community chaplains and spiritual counselors to meet patient and family needs.
• Provide funeral or memorial services as requested and plan periodic memorials for staff and volunteers.
• Supervise spiritual care volunteers when assigned to patients and families.
