Angels Care Hospice is seeking a Hospice Chaplain to deliver spiritual care to patients, families, and caregivers, directly or through coordination with other spiritual counselors. You will participate in interdisciplinary care planning and quality improvement activities.

The role includes recruiting community chaplains and providing education and support to the care team, as well as offering funeral or memorial services when requested.

Qualificações

• Graduate of accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).

Responsabilidades

• Provides spiritual care to patients, families, and caregivers.
• Coordinates care with the interdisciplinary team.
• Recruits community chaplains and spiritual care volunteers as needed.
• Documents services and participates in quality improvement activities.
• Provides funeral or memorial services on request.
• Attends patient care conferences and supports education for the team.
