Job summary

HIA Hospice in Grand Forks, ND seeks a Chaplain to provide spiritual care to patients and families within the interdisciplinary team framework. This 1.0 FTE role is based in the Grand Forks office and surrounding communities.

Responsibilities include assessing spiritual needs, developing care plans, delivering chaplain visits, documenting interactions, and coordinating with IDGs and the pastoral community. Must be ordained or licensed; Master’s in Divinity preferred, CPE desirable.

Qualifications

• Must be ordained, ordered, or licensed by a recognized denomination; Master’s in Divinity strongly preferred.
• Clinical Pastoral Education (CPE) desirable.
• Strong interpersonal communication and teamwork skills; commitment to palliative and hospice care.

Responsibilities

• Assess patient/family need for spiritual care and plan of care.
• Develop a written plan of care with patient/family and hospice IDG.
• Provide chaplaincy visits to patient/family as per plan.
• Document all contacts and interventions promptly.
• Provide patient status updates to the IDG.
• Collaborate with pastoral community to ensure continuity of care.
• Adhere to contracts with other providers and HIPAA policies.
• Participate in IDGs, family support and staff meetings.
