Qualifications

• Bachelor's degree in religious studies and a master's in theology or pastoral studies.
• Certification is preferred; ordination or commissioning and a letter of endorsement may be required.
• Commitment to ongoing learning and completing CPE/education.
• Hospice care experience is preferred.
• Comfort in an interfaith setting.
• Ability to network with community clergy and congregations.
• Knowledge of hospice philosophy of care.
• Strong verbal and written communication and organization skills.
• Contingent upon satisfactory references and criminal background checks.
• Must possess a driving license with a valid vehicle and CPR certification.

Responsibilities

• Assesses spiritual needs of patients and families and ensures timely services.
• Participates in comprehensive assessment to develop the plan of care for spiritual needs.
• Provides direct spiritual care to patients and families.
• Serves as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and policy review.
• Documents direct services and ongoing communication with chaplains and counselors.
• Attends patient care conferences as a member of the IDG.
• Provides consultation and support to the IDG on spiritual care.
• Recruits community chaplains and volunteers to meet needs; educates congregations about hospice care.
• Provides funeral or memorial services as requested.
• Plans memorial services for staff, volunteers, and community clergy.
• Assists in supervising spiritual care volunteers with patients/families.
