Job summary

Qualifications

Responsibilities

• Assures spiritual assessment of patients and families; delivers timely services.
• Develops individualized plan of care for spiritual needs through comprehensive assessment.
• Provides direct spiritual care to patients and families.
• Liaises with community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and policy review.
• Documents services and communicates with community chaplains.
• Attends IDG meetings; provides consultation to the team on spiritual care.
• Recruits community chaplains and volunteers; conducts education in congregations.
• Plans memorial services and provides for patient/family needs as requested.
