Qualifications

• Ability to provide spiritual care to diverse faiths and backgrounds.

Responsibilities

• Provide spiritual care and emotional support to hospice patients and families.
• Conduct spiritual assessments and develop individualized care plans.
• Offer prayer, counseling, and grief support per patient beliefs and preferences.
• Collaborate with nurses, social workers, physicians, and hospice team for holistic care.
• Support families through end-of-life transitions and bereavement processes.
