Job summary

Moments Hospice seeks a Chaplain to provide spiritual care to patients and families navigating end-of-life journeys. You will conduct spiritual assessments and develop care plans, collaborating with an interdisciplinary team to ensure holistic support across diverse beliefs.

Qualifications

• Master's degree in divinity (MDiv) or equivalent theological degree.
• Experience working with death and dying individuals and families.
• Comfort in interfaith settings and ability to accept diverse beliefs.
• Strong teamwork and communication skills.

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provide direct spiritual care to patients and families.
• Collaborate with community clergy and spiritual counselors.
• Maintain accurate spiritual care service records.
• Attend team meetings and offer spiritual care expertise.
• Recruit and supervise spiritual care volunteers.
• Participate in quality improvement initiatives.
