Job summary

Moments Hospice is seeking a Chaplain to provide spiritual support to patients and families navigating end-of-life journeys. You will collaborate with an interdisciplinary team to ensure holistic care that respects diverse beliefs and values.

The role requires a Master’s degree in divinity, a valid driver’s license and auto insurance. Responsibilities include spiritual assessments, direct care, records, team meetings, and mentoring volunteers. A company car with fuel and insurance is included.

Qualifications

• Master's degree in divinity (MDiv) or equivalent theological degree (required).
• Experience working with death, dying individuals, and their families.
• Comfort in interfaith settings and ability to accept diverse beliefs.
• Strong teamwork and communication skills.
• Valid driver's license (required).
• Valid Auto Insurance (required).

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provide direct spiritual care to patients and families.
• Collaborate with community clergy and spiritual counselors.
• Maintain accurate spiritual care service records.
• Attend team meetings and offer spiritual care expertise.
• Recruit and supervise spiritual care volunteers.
• Participate in quality improvement initiatives.
