Job summary

Moments Hospice is seeking a Chaplain to provide spiritual support to patients and families facing end-of-life journeys. You will conduct spiritual assessments, develop care plans, and collaborate with an interdisciplinary team.

This on-the-road role includes a generous benefits package and autonomy in spiritual care, with on-call support and opportunities for growth. Requirements include a Master's degree in divinity, experience with death/dying and strong teamwork, plus a valid driver's

Qualifications

• Master's degree in divinity (MDiv) or equivalent theological degree (required)
• Experience working with death, dying individuals, and their families
• Comfort in interfaith settings and ability to accept diverse beliefs
• Strong teamwork and communication skills
• Knowledge of hospice philosophy and experience preferred (preferred)
• Valid driver's license (required)
• Valid Auto Insurance (required)

Responsibilities

• Conduct spiritual assessments and develop individualized care plans
• Provide direct spiritual care to patients and families
• Collaborate with community clergy and spiritual counselors
• Maintain accurate spiritual care service records
• Attend team meetings and offer spiritual care expertise
• Recruit and supervise spiritual care volunteers
• Participate in quality improvement initiatives
