Job summary

Moments Hospice is seeking a Chaplain to provide spiritual support to patients and families, traveling as needed. You will collaborate with an interdisciplinary team to ensure compassionate end-of-life care and participate in quality initiatives.

Qualifications

• MDiv or equivalent theological degree (required).
• Experience with death and bereavement in families.
• Comfort in interfaith settings and respect for diverse beliefs.
• Strong teamwork and communication skills.

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provide direct spiritual care to patients and families.
• Collaborate with community clergy and spiritual counselors.
• Maintain accurate spiritual care service records.
• Attend team meetings and offer spiritual care expertise.
• Recruit and supervise spiritual care volunteers.
• Participate in quality improvement initiatives.

• Experience working with death, dying individuals, and their families

• Knowledge of hospice philosophy and experience preferred (preferred)
• Valid driver's license (required)
• Valid Auto Insurance (required)
