Role Responsibilities:

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
• Help the patient maintain good personal hygiene.
• Assist in maintaining a safe and healthy environment.
• Prepare and serve light meals.
• Assist in feeding patients when needed.
• Assist the patient in ambulation and with the safe use of mobility aids.
• Perform light housekeeping tasks such as emptying trash and changing linens.
• Assist with treatments as ordered and permissible by state regulations.
• Promote patient mental alertness through engaging activities.
• Provide emotional and psychological support.
• Report changes in patient condition or home situation.
• Demonstrate care techniques to primary caregivers.
• Practice infection control and standard precautions.
• Accurately document visits and participate in staff meetings.
• Participate in quality improvement activities and complete documentation promptly.

Candidate Requirements:

• Compassionate, kind, and empathetic nature.
• Comfortable providing care wherever the patient calls "home".
• Ability to remain flexible and composed in crisis situations.
• Certified Nursing Assistant (CNA) license.
• Six months of patient care experience preferred.
• Current driver’s license, automobile insurance, and CPR certification.
