Role Responsibilities:
- Report directly to the Administrator, Executive Director, or Patient Care Manager and function under the supervision of the Registered Nurse and/or Clinical Supervisor.
- Help the patient maintain good personal hygiene.
- Assist in maintaining a safe and healthy environment.
- Prepare and serve light meals and assist with feeding patients when needed.
- Assist in ambulation and with the safe use of mobility aids.
- Perform light housekeeping tasks such as trash removal and linen changes.
- Assist with treatments as ordered and allowable by state regulations.
- Promote mental alertness through engagement in activities.
- Provide emotional and psychological support to patients and families.
- Report changes in patient condition or home situation to the nursing team.
- Demonstrate basic care techniques to primary caregivers.
- Practice infection control and standard precautions.
- Document visits accurately and participate in staff meetings and quality improvement activities.
Qualifications:
- Compassionate, kind, and empathetic attitude.
- Comfortable providing care in a patient's home environment.
- Flexible and composed in crisis situations.
- Certified Nursing Assistant (CNA) license.
- Six months of patient care experience preferred.
- Additional experience in hospital or emergency settings advantageous.
- Valid driver’s license and auto insurance.
- Current CPR certification.
