Optimal Care is seeking a compassionate hospice/home health aide to travel to patient homes and facilities within the assigned service area. You will assist with activities of daily living, safety transfers, meals, and vital signs, while following individualized care plans and documenting care provided.
Applicants should have a current nurse aide certification, 1 year of relevant experience preferred, and a valid driver’s license with reliable transportation.
Full Time: Monday‑Friday, 8:00 AM - 5:00 PM, with weekend rotation
Part Time: Three weekdays (8:00 AM - 5:00 PM), with weekend rotation
Per Diem: Flexible weekdays, Monday‑Friday (8:00 AM - 5:00 PM), with weekend rotation
$18—$22 USD
