Job summary

Elevation Hospice of Colorado is seeking a compassionate Certified Nursing Assistant (CNA) to join our hospice team in Colorado. The position focuses on providing personal healthcare and supportive services in the patient’s residence, under the supervision of a registered nurse.

The role requires CNA licensure and state-approved nurse aide training, with reliable transportation. You will work closely with patients and families to ensure comfort, dignity, and high-quality care in a home setting.

Qualifications

• High School Diploma preferred.
• Must have CNA license.
• Must have Nurse aide training/competency program approved by the state.
• Reliable transportation. Valid and current auto liability insurance.

Responsibilities

• Provide personal healthcare and related services to the patient in their place of residence, assisting in maintaining a safe and clean environment.
• Work cooperatively with patient, family, and supervisor; observe and report findings to the supervising RN.

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
