Job summary

Three Oaks Hospice is seeking a compassionate Certified Nursing Assistant to join our team serving Longmont and surrounding communities. You will provide bedside care in patients’ homes under RN supervision, with a Monday–Friday 8:30am–5pm schedule and a territory including Longmont, Erie, Frederick, and Boulder.

The role requires CNA certification, a valid driver’s license, and reliable transportation. Previous hospice or nursing assistant experience is preferred.

Qualifications

• High School Diploma Preferred.
• State-approved nurse aide training/competency program.
• Must have CNA license.
• Must have current driver’s license.
• At least one year experience as Hospice Aide or Nursing Assistant.
• Reliable transportation and auto insurance.

Responsibilities

• Provide personal healthcare and related services in the patient’s residence.
• Assist in creating a safe, clean living environment and coordinate with the supervising RN.

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY:

Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

Qualifications

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
