Job summary

St. Mary's Home Care & Hospice in Athens, GA is hiring a Full-time Certified Home Health Aide/Hospice (CNA) to provide one-to-one in-home care under RN supervision. You will follow care plans, assist with personal care, and monitor vital signs with accurate documentation.

We offer a $2,000 sign-on bonus, mileage reimbursement, daily pay, comprehensive orientation, and opportunities for advancement within a compassionate, mission-driven team.

Qualifications

• Completion of CNA 75 hr course and state certification.
• Ability to follow care plans developed by RN and document patient data.
• Excellent bedside manners and ability to maintain professional boundaries.

Responsibilities

• Follow care plan developed by RN.
• Report client changes in condition to RN and document in chart.
• Assist clients with personal care (bathing, grooming, etc.) and basic daily activities.
• Assist with household tasks (meals, light housekeeping, linens, laundry).
• Measure and record vital signs as ordered.
• Support administration of medications under RN supervision (no med administration).

C.N.A./Hospice/Home Care - Home Health Aide Position Purpose

Our hospice/home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

What You Will Do

• Follows care plan developed by RN.
• Reports client changes in condition to RN, as described on care plan.
• Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
• Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
• Measures and records client’s oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
• Performs range of motion and other simple procedures as an extension of therapy services as assigned.
• Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind

Minimum Qualifications

• High School diploma or GED required.
• Completion of CNA 75 hr course.
• Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
• Documentation of one year’s employment as an aide in an institution, home care, or hospice setting.
• Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
• Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
• Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis.
• Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics and adhere to the Compliance Program.
• Must have current Driver’s license and reliable transportation to and from work site.
