Job summary

Montage Health is seeking a Hospice Case Manager RN to ensure smooth operations of the hospice office, handling informational calls, coordinating with MDs, and guiding families and patients in hospice care. The role combines phone support with in-person care as needed.

Requires CA RN license, BLS, and a minimum of three years nursing experience, including two years in home hospice in the last five years; Bachelor's degree in nursing preferred. Per diem shifts and driving expectations apply.

Qualifications

• Three years as a nurse is required.
• A minimum of two years home hospice experience within the last five years is required.
• Spanish speaking preferred.

Responsibilities

• Field informational calls that require an RN and speak with MDs.
• Take telephone orders and assist nurses in the field.
• Inform and instruct families and patients on hospice care.
• Make adjustments in visit schedules and schedule the nurse practitioner.
• Document and complete re-certifications.
• Notify Assistant Director or Director of any problems.
