Qualifications

• Current State Registered Nurse license.
• Valid driver's license and reliable transportation with auto liability insurance.
• Two years of hospice nursing experience in clinical care or home health.
• Ability to multi-task in a multidisciplinary setting.
• Strong organizational, interpersonal and communication abilities.

Responsibilities

• Direct and coordinate clinical departments and sites, ensuring quality, continuity and safety in line with regulations.
• Supervise nursing staff and team members to maintain plan of care and continuity of services.
• Provide education and development for staff; conduct timely evaluations.
• Coach, give feedback and perform annual performance reviews.
• Ensure timely submission of all documentation.
• Perform other duties as required and ensure compliance with laws and policies.
