Job summary

Lehigh Valley Health Network is seeking a Registered Nurse (RN) Hospice Case Manager for a full-time day shift serving patients in the home setting. You will coordinate care with an interdisciplinary team, determine care levels, and implement skilled services under the physician plan of care.

You will collect clinical data for CMS and support patient/family education on end-of-life care, advance directives, and discharge planning.

Qualifications

• Active RN license in PA.
• BLS certification required.
• Experience in home health or hospice preferred.

Responsibilities

• Coordinate and direct care for patients in the home setting with an interdisciplinary team.
• Develop, implement, and revise the plan of care in collaboration with physicians.
• Collect data per CMS regulations to reflect outcomes and reimbursement.
• Provide patient/family education on end-of-life, hospice, and care planning.
• Communicate changes in patient status to the care team promptly.
