Job summary

MJHS Hospice & Palliative Care offers comprehensive services in inpatient facilities and community settings. The Hospice Care Planner will evaluate referrals, establish care plans, and facilitate admission from hospital discharge to an appropriate hospice program.

As a key member of the business development and marketing team, you will build relationships with physicians, social workers, case managers and discharge planners to coordinate patient care and family support.

Qualifications

• Licensed as Registered Nurse with NYS registration.
• Minimum two years hospice nursing experience.
• Excellent written and verbal communication skills.
• Bilingual Spanish/English in some locations.

Responsibilities

• Evaluate and establish plan of care for patients referred to Hospice Care services.
• Facilitate admission of referrals from hospital discharge to appropriate hospice program.
• Develop and maintain relationships with physicians, social workers, case managers and discharge planners.
