Job summary

Hearth Hospice in Chattanooga is seeking a Patient Care Advocate to support our Liaisons, identify referral sources, and educate families about hospice services. You will use hospice knowledge and marketing skills to engage with referral sources, families, and prospective patients, helping to grow program census and preferred hospice services.

Schedule is Monday–Friday, 8 am–4:30 pm in the Chattanooga territory.

Qualifications

• Bachelor’s degree preferred; experience may substitute education.
• Minimum 1 year in hospice required; 2–3 years preferred.
• Prior customer service or sales experience strongly preferred.

Responsibilities

• Collaborate with Liaisons to gather patient information and provide admissions support.
• Participate in community events to build brand awareness and census growth.
• Meet patients and families to introduce hospice services with compassionate care.
• Ensure accurate CRM documentation and timely marketing reports.
• Identify key accounts and communicate insights to Liaisons and Regional Sales Director.
