Job summary

Heart to Heart Hospice seeks a Customer Service Liaison to promote a patient-centered hospice presence across healthcare facilities, including assisted living, nursing homes, home health, and hospitals.

The role coordinates care among healthcare professionals, communicates with patients and families to ensure needs are met and satisfaction remains high, and supports staff and leadership in growth efforts.

Qualifications

• 2-3 years of healthcare experience.
• Excellent customer service skills.
• Hospice, home health, nursing home, assisted living, or hospital experience preferred.
• Current State Driver’s License.
• Reliable transportation with active auto liability insurance.
• Ability to establish and maintain effective working relationships with staff, facilities, the public, the Board of Directors and Medical Director.
• Demonstrates good communication and interpersonal skills.
• Experience with EMR software is a plus.

Responsibilities

• Promote a customer-focused hospice presence in the healthcare community across assisted living facilities, nursing homes, home health, and hospitals.
• Coordinate care amongst healthcare professionals.
• Communicate weekly (daily if needed) with patients and families to ensure all needs are met and Heart to Heart delivers high satisfaction.
• Identify problems or dissatisfaction and ensure quick resolutions while informing clinical team and management as needed.
• Coordinate and provide in-services or educational events for family members and facilities.
• Support agency leadership to meet growth and development targets.
• Be flexible with after-hours and weekend consultations as referrals require.
• Ensure compliance with local, state, and federal laws, Medicare regulations, and policies.
