Job summary

Compassus is seeking a Hospice Team Coordinator to oversee care delivery, data management, and coordination across the interdisciplinary team. The role emphasizes accurate record-keeping, communication with families and physicians, and ensuring compliant documentation in HCHB and EMR systems.

The position requires attention to detail, strong English communication, and experience with medical records or office support.

Qualifications

• Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, fractions, and decimals.
• Excellent written and verbal communication; able to present information to leaders and external parties.
• Maintains confidentiality of patient records and HIPAA-compliant handling of information.

Responsibilities

• Accurately enters and manages patient data, including referrals and demographics.
• Ensures admission documentation is complete and uploaded into HCHB.
• Maintains patient records in HCHB and EMR systems; generates regulatory reports.
• Monitors DME and supplies orders and informs management of trends.
• Monitors timely return of physician orders and processes them for care.
• Verifies and uploads paper visit records into EMR and HCHB for complete histories.
• Keeps patient charts well organized for easy retrieval.
• Ensures care level matches billed level to support accurate reimbursement.
• Collects decertification paperwork and notifies providers of discharges.
• Initiates medical records requests and provider notifications as needed.
• Facilitates coordination among IDT for holistic patient care.
• Acts as central contact for hospice team, patients, and families.
• Handles calls, takes messages, and routes them to the appropriate staff.
• Manages phone system and secondary system when needed.
• Assists DCS with pre/post-IDT activities and prepares UPOCs; sends to physicians.
• Prepares patient binders in facilities and keeps essential information organized.
• Performs other duties as assigned.
