Job summary

Next Care Hospice is seeking a Patient Care Coordinator to serve as the administrative link between patients, families, and the interdisciplinary care team. This non-clinical role coordinates logistics, manages scheduling, and ensures care delivery aligns with patient plans and regulatory requirements.

You will support enrollment, documentation, and communications across care teams, helping families navigate hospice services with empathy and clarity while maintaining accurate EMR records.

Qualifications

• High school diploma or GED required; Associates or Bachelor's preferred.
• Minimum 2 years of administrative or coordination experience in healthcare.
• Experience with EMR systems required.
• Hospice, home health, or palliative care experience preferred.

Responsibilities

• Coordinate and schedule visits for the interdisciplinary team.
• Manage patient care schedules and update EMR in real time.
• Communicate schedule changes to staff and families.
• Prepare intake for new patients and gather required documentation.
• Assist with billing verification and reports for meetings.
