Job summary

Chapters Health System seeks a Bereavement Counselor to provide individual and group counseling for hospice patients, families, and community members. You will document contacts, care plans, and maintain compliant files while supporting flexible hours, including some weeknights and weekends.

Role requires a master’s degree in counseling or related field (or BA with prior experience) and adherence to HIPAA and licensure requirements. Travel may be needed to other locations as required.

Qualifications

• Master’s degree in counseling, social work, marriage and family therapy, or related field required if hired after 5/26/2024.
• Bachelor’s degree in counseling, social work, marriage and family therapy, or related field preferred if hired before 5/26/2024.
• Clear Level 2 background check.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Professional licensure as required by state where care is provided.

Responsibilities

• Provide individual and group counseling for hospice patients and families.
• Document client contacts and care plans timely and accurately.
• Maintain legally required files according to state, federal law and policy.
• Offer crisis intervention and counseling on a flexible schedule including some weeknights and weekends.
• Collaborate with Interdisciplinary Team to ensure quality services.
• Attend meetings and participate in program improvement initiatives.
