Qualifications

• Master's degree in counseling, social work, marriage and family therapy, or related field required if hired after 5/26/2024.
• Bachelor's degree in counseling, social work, marriage and family therapy, or related field preferred if hired before 5/26/2024.
• Clear Level 2 background check required.
• Must possess a valid driver's license, current auto insurance and reliable transportation. Licensure as required by state.

Responsibilities

• Provide individual and group counseling and therapy for hospice patients, family members and community individuals who have experienced loss.
• Document client contacts and care plans in a timely manner; maintain required files.
• Collaborate with Interdisciplinary Team to ensure high-quality services.
• Be available for on-call crisis intervention and flexible scheduling including some weeknights and weekends.
