VNA Health Group in West Orange, NJ seeks an Assistant RN Clinical Manager Hospice to support the Hospice team and oversee day-to-day clinical operations alongside the Clinical Manager. The role focuses on coordinating patient care and ensuring high-quality hospice services delivered at home or in community settings.

Ideal candidates are RNs with BSN, leadership experience, strong communication and computer skills, and a background in hospice or oncology.

Qualifications

• RN with BSN; oncology/hospice and palliative care certification preferred.
• 2–3 years of experience with community health, oncology, or hospice nursing preferred.
• Leadership or supervisory experience preferred.
• Excellent verbal and written communication skills.
• Strong computer skills.
• Excellent organizational and interpersonal skills.

Responsabilités

• Works closely with the Clinical Manager to support the day-to-day clinical operations of the Hospice team
• Assists with coordinating patient care and ensuring continuity of services for hospice patients and their families
• Provides clinical guidance and support to Hospice team members
• Assists with staff supervision in conjunction with the Clinical Manager and the organization’s managerial and leadership practices
• Helps monitor patient care needs, staff assignments, and overall team workflow
• Manages a caseload of hospice patients as needed
• Participates in team meetings and interdisciplinary coordination of care
• Supports community outreach activities to improve access to hospice care
• Adheres to agency infection control and safety policies
• Supports the organization’s mission by striving for excellence in all aspects of the position and maintaining positive working relationships with colleagues
