Responsibilities include assisting with bathing, feeding, vital signs, mobility, and reporting changes to the Case Manager; requires STNA/CNA certification and CPR; team-based approach with strong communication skills.

Qualifications

• CNA certification required by state for patient care.
• CPR certification required.
• Strong communication and interpersonal skills essential.

Responsibilities

• Provide personal care and supportive services per patient care plan.
• Assist with bathing, feeding, grooming, and mobility.
• Take and record basic vital signs and report changes to Case Manager.
