Job summary

Day City Hospice seeks a Hospice Aide to provide personal care and supportive services to patients, following each patient’s care plan. The role emphasizes comfort, dignity, and family support within a locally operated hospice network.

Qualifications

• Current CPR certification
• Strong communication, observation, and interpersonal skills
• Ability to work independently while collaborating with the interdisciplinary team
• Understanding of hospice philosophy and comfort with end-of-life care
• STNA/CNA certification current and eligible to provide patient care

Responsibilities

• Provide personal care services promptly and compassionately based on patient assignments and individual needs
• Ensure patient safety and properly utilize equipment such as oxygen, side rails, foot stools, and other assistive devices
• Assist with bathing, oral hygiene, shampoos, back rubs, and changing bed linens as assigned
• Assist patients with dressing and undressing
• Plan and prepare nutritious meals, including grocery shopping when assigned
• Assist patients with feeding as needed
• Take and record temperature, pulse, respirations, and blood pressure as ordered and within demonstrated competency
• Observe and provide appropriate skin care to help prevent breakdown and maintain patient comfort
• Report patient condition, concerns, and significant changes to the Case Manager
• Maintain awareness of caregiver dynamics and communicate relevant concerns to the care team
• Provide respite support for family members and caregivers when appropriate
