Position Details

Hospice Aide - Full Time

Location: Nashville, Tennessee

Status: Full Time

Hours: Monday - Friday, 8 a.m. - 5 p.m.

Summary: The Home Hospice Aide is a paraprofessional member of the hospice care team trained to provide personal care and other unskilled services in the patient's residential setting under the supervision of a registered nurse. Performs various personal care services to promote patient comfort and safety.

Essential Duties and Responsibilities

1. Assist patients with Activities of Daily Living (ADL), including personal care, hygiene, bathing, skin and nail care, shaving, eating, dressing, elimination, ambulation, transfers, bed-making, light housekeeping, and meal preparation as directed by care plan.
2. Provide emotional support to patients and caregivers.
3. Maintain a safe and clean patient environment, using equipment and supplies properly.
4. Take and record patient vital signs as advised.
5. Report changes in patient condition to RN or Clinical Coordinator.
6. Document activities and observations accurately and submit according to agency procedures.
7. Follow the care plan developed by the RN.
8. Be available via agency cell phone from 8:00 A.M. to 5:00 P.M. on scheduled days.
9. Perform other duties as assigned.

Minimum Qualifications

• High school diploma or GED.
• At least one year of direct patient care experience, preferably in home care.
• Professional maturity and ability to respond calmly in stressful situations.
• Current Tennessee CNA certification.
• Valid driver’s license and automobile insurance if required for duties.
